2025-26 School Year Tuition Program Applications

About our Program:

Thank you for your interest in Sunrise Preschool! We are a Provo School District preschool program open to all Provo City residents. We accept children ages 3-4 of all abilities and developmental levels, and have preschool students attending our Special Education and Tuition programs in inclusive classrooms.

Our sessions are 2.5 hours, with morning and afternoon sessions available

Session Times

• AM Session: 9:00 – 11:30
• PM Session: 12:30 – 3:00

Class Sessions

• Students who turn 3 after September 1, 2025 attend 1 day/week
  • Beginning after his or her 3rd birthday
• Students who are age 3 prior to September 1, 2025 attend 2 days/week
  • Mondays & Wednesdays  OR  Tuesdays & Thursdays
• Students who are age 4 prior to September 1, 2025 attend 4 days/week
  • Monday through Thursday

Tuition pricing for the 2025-2026 school year:

• $40/month for our 1 day/week tuition program
• $80/month for our 2 day/week tuition program
• $160/month for our 4 day/week tuition program

Tentative First Day of School: August 18, 2025

Tentative Last Day of School: May 15, 2026

To apply for the tuition preschool, follow the instructions listed below. If you are applying for multiple children, you will need to complete each of the steps for each child. Please do not pay the $20 registration fee until after you have been notified of acceptance.

Application Instructions:

1. Complete the first portion of the online application
   • Go to forms.provo.edu and click on the ‘Pre-School’ link. Complete the ‘New Student Preregistration’ form and click ‘Submit Information’ at the bottom of the page.
2. Complete the second portion of the online application.
   • Sunrise Preschool will receive an email that the first portion of the online application is submitted and our secretary will create a Powerschool account for you. You will then receive an email from rosaf@provo.edu with instructions to complete the PowerSchool registration.
3. Submit all required documents (May be completed during Step 2)
   • Upload the following documents as part of the online application OR email them to our secretary at rosaf@provo.edu
   • A copy of your child’s birth certificate
   • An immunization record that meets the state guidelines for preschool students or an exemption.
     • Required Immunizations
   • Proof of address (Utility bill from the last 30 days or rental agreement form)
     • If you live outside of Provo City School District boundaries, you may apply for school choice through Student Services, although this does not guarantee acceptance.
   • Picture ID(s) of parents/guardians
4. Indicate your session preference by completing this form
5. After you have been notified of your child’s acceptance, pay the $20 registration fee to secure your child’s spot
