Utah School Registration Immunization Requirements
Utah State Law requires that all students submit a completed immunization record to the school before the first day of school attendance. These requirements are in effect for the 2021-2022 school year in all Utah public and private schools.
A student must have proof of the following immunizations for school enrollment or submit one of the exemption forms listed below.
Preschool Immunization Requirements
- 4 doses of DTP/Dtap/DT
- 3 doses of Polio (IPV)
- 1 dose of MMR
- 3 doses of Hepatitis B (HBV)
- 2 doses of Hepatitis A (HAV)
- 1 dose of Varicella (Chickenpox) (or a history of the disease documented and signed by a healthcare provider)
- HIB – doses adequate for age
- Prevnar (Pneumonia) – doses adequate for age, including one dose of Prevnar 13
K-6th Grade Immunization Requirements
- 5 doses of DTP/Dtap/DT/Tdap (4 doses are acceptable if the 4th dose was given after the 4th birthday; 3 doses are acceptable if the 3rd dose was given after the 7th birthday)
- 4 doses of Polio (IPV) (Last dose must be given after the 4th birthday or 5 doses are required; 3 doses are acceptable if the 3rd dose was given after the 4th birthday)
- 3 doses of Hepatitis B (HBV) (Last dose must be given after 6 months of age or 4 doses required)
- 2 doses of Varicella (Chickenpox) (History of disease must be documented and signed by a healthcare provider)
- 2 doses of Hepatitis A (HAV)
7th-12th Grade Immunization Requirements
- 5 doses of DTP/Dtap/DT/DTP (4 doses are acceptable if the 4th dose was given after the 4th birthday; 3 doses are acceptable if the 3rd dose was given after the 7th birthday)
- 1 dose of Tdap (Tetanus, Diphtheria, Pertussis)
- 3 doses of Polio (3rd dose must be given after the 4th birthday)
- 3 doses of Hepatitis B (HBV) (Last dose must be given after 6 months of age or 4 doses required)
- 2 doses of Varicella (Chickenpox) (History of disease must be documented and signed by a healthcare provider)
- 2 doses of Hepatitis A (HAV)
- 1 dose of Meningococcal
Note: The first dose of MMR, Varicella, and Hepatitis A must be given after the first birthday to be considered a valid dose.
Exemptions
Utah law allows for the following immunization exemptions:
- Personal Exemption
- Religious Exemption
- Medical Exemption
To claim an exemption, parents or guardians must complete an official exemption form, which can be obtained from the Utah Department of Health or the school district.
